Daytime sleepiness, functionality, and stress levels in chronic neck pain and effects of physical medicine and rehabilitation therapies on these situations

INTRODUCTION: To evaluate the relationship between symptom severity, daytime sleepiness, and perceived stress levels and the impact of physical medicine & rehabilitation (PMR) therapies on these situations in chronic neck pain (CNP) conditions.
METHODS: The study included 54 patients with CNP and 20 healthy control individuals. Patients with CNP were divided into two groups: the PMR therapy group (n=34) and the CNP control group (n=20). The PMR therapy programs of the patients included TENS, hot packs, therapeutic ultrasound, and exercises. Visual analog scale (VAS) at activity and resting for neck pain, Neck Disability Index (NDI), Perceived Stress Scale (PSS), Epworth Sleepiness Scale, chin-manubrium distances (CMD), and tragus-wall distances (TWD) values were evaluated before and after the treatment programs.
RESULTS: Significant differences were found between the CNP patients and healthy controls regarding PSS, TWD, and CMD values. Furthermore, significant differences were detected between the PMR group and the CNP control group in the final evaluation of the VASresting, VASactivity, PSS, and NDI levels.
DISCUSSION AND CONCLUSION: Evaluation of CNP from a single point of view can leave clinically missing points. Patients with CNP should be assessed for daytime sleepiness, stress levels, and functionality, and PMR therapies can be effective in relieving pain and psychological stress in patients with CNP.

Kronik Boyun Ağrısında Gündüz Uykuluk Hali, Stres Düzeyleri, Fonksiyonellik ve Fiziksel Tıp ve Rehabilitasyon Tedavilerinin Bu Durumlar Üzerine Olan Etkisi

GİRİŞ ve AMAÇ: Kronik boyun ağrısı (KBA) ile algılanan stres düzeyleri, gündüz uykululuk hali, semptom şiddeti ilişkilerini incelemek ve fiziksel tıp ve rehabilitasyon (FTR) tedavilerinin bu durumlar üzerine olan etkilerini incelemek amaçlanmıştır.
YÖNTEM ve GEREÇLER: Çalışmaya 54 KBA’ sı olan hasta ve 20 sağlıklı kontrol dahil edilmiştir. KBA’ sı olan hastalar, FTR tedavi grubu (n=34) ve kontrol KBA grubu (n=20) olarak ikiye ayrılmıştır. FTR tedavi programları TENS, hot pack uygulama, terapötik ultrason ve egzersizleri içermekteydi. Boyun ağrısı için hareket ve istirahat görsel ağrı skorları (GAS), Epworth uykuluk hali (EUH), algılanan stres ölçeği (ASÖ), boyun özürlülük indeksi (BÖİ), tragus-duvar mesafesi (TDM), çene-sternum mesafesi (ÇSM) değerleri tedavi öncesi ve sonrası incelenmiştir.
BULGULAR: ASÖ, TDM, ÇSM değerlerinde sağlıklı kontrol grubu ile KBA’ sı olan hastalar arasında istatistiksel olarak belirgin fark saptanmıştır. FTR grubu ile kontrol KBA grubu arasında son değerlendirmede GAS hareket ve istirahat, ASÖ, BÖİ değerleri açısından belirgin farklılık saptanmıştır.
TARTIŞMA ve SONUÇ: KBA’ na tek açıdan bakmak klinik olarak eksik noktalar bırakabilir. KBA’ sı olan hastalar stres düzeyleri, gündüz uykululuk hali, fonksiyonellik açısından değerlendirilmelidir ve FTR tedavileri erken dönem sonuçlarında boyun ağrısını azaltmakla beraber eşlik eden bu tür problemlerin giderilmesi içinde faydalı olabilir.
